Exploring the use of Silver Diamine Fluoride (SDF) on patients at the University of Utah dental clinics

Dental caries is among the most common chronic infections of childhood, the primary cause of which is bacterial. The biofilm adheres to the outer layer of the tooth metabolizing dietary sugars. which produces acid that demineralizes weakens tooth structure. When decay becomes severe, dentists use surgical intervention to remove the decay. However, this is a costly, invasive procedure that requires the removal of tooth structure. Silver Diamine Fluoride (SDF) is a caries-arresting treatment that is applied topically and does not require the removal of tooth structure. Silver's antimicrobial properties, combined with fluoride offer a natural tooth repair process through remineralization. Application of SDF is a quick, painless, cost-effective treatment but does cause black staining on the carious lesions, creating cosmetic concerns for patients and providers. Literature suggests dentists lack knowledge about this treatment option and its potential benefits to patients. Our study aims to understand why patients and providers choices around SDF. We are conducting a retrospective analysis of patients presenting to Univerisity of Utah dental clinics and for whom SDF was applied. Data on the clinical status is abstracted from patient charts, including clinical oral health status, payor type, and visit attendance, and then analyzed to discover trends. We hypothesize that patients who are un- or underinsured and/or lack regular engagement with oral healthcare services will have a higher probability of SDF applications due to limited access and ease of treatment. Results will help to inform treatment recommendations for use of SDF for both patients and providers.
